Vaccination Records -OR- Religious Exemption Letters:

*Please provide us with an up-to-date vaccination record/chart or religious exemption letter prior to your child’s first day. Returning students must provide a new/updated record or letter each season. We DO NOT require COVID or FLU vaccines.

*Vaccination records or exemption letters may be dropped off, faxed (732-223-0288) or emailed (siltonvax@gmail.com) to our office or mailed in along with your child’s registration prior to their first day of camp.
