Medical History & Required Documents

Silton Swim School is required to have a Health Director on staff at all times. This adult is either a registered nurse, paramedic, or emergency medical technician. If you prefer to speak to our Health Director prior to your child’s first day, please call our office and we can make arrangements. Our Health Director’s office is stationed pool side and students have access to first aid equipment and supplies, including emergency equipment, an AED, and other necessary medicals needs at any time during camp. In addition, all instructors, age 15 & older, have completed and passed The American Red Cross’ Lifeguard, First Aid, and CPR/AED certification course.

Permission to Dispense Medication:

If your child requires ANY medication while attending Silton Swim School, you are REQUIRED to complete our MEDICATION DISPENSING FORM prior to your child’s first day of camp. We are required to obtain this document in order to dispense ANY medications taken routinely or temporarily. This document requires BOTH A PARENT AND A PHYSICIANS AUTHORIZATION. Medication will not be given without both authorizations. *CHILDREN WILL NOT BE ALLOWED TO PARTICIPATE AT SILTON UNLESS ALL REQUIRED DOCUMENTATION IS FULLY COMPLETE AND IN OUR OFFICE.

ALL MEDICATION (PRESCRIPTION OR OVER-THE-COUNTER) MUST BE LABELED WITH YOUR CHILD’S NAME AND STORED IN OUR OFFICE WITH THE ORIGINAL PRESCRIPTION LABELED CONTAINER IN ACCORDANCE WITH THE MANUFACTURER INSTRUCTIONS.*Please do NOT leave any medication or EPI-PENS in your child’s bag or with their instructors.

History of Seizures:

If your child has ANY history of SEIZURES, we require a doctor’s note stating that your child can participate in GROUP swimming activity/lessons without 1:1 instruction. This note can be written or emailed to us during the time of enrollment, or any time prior to your child’s first day of camp.

Complete our mandatory Action Care Plan for Seizures if your child is under the care of a physician for his/her seizures. **If your child is no longer under the care of a physician for seizures, please provide us a note/email starting that.

Asthma:

Complete our mandatory Action Care Plan for Asthma if your child has Asthma. *All inhalers or medications must be stored in our office and labeled with your child’s name in their original prescription labeled container.

Food & Bee Allergies & Students Requiring an EPI-PEN:

Silton Swim School is a FOOD FREE facility during lesson hours. As an extra pre-caution, please complete our FOOD ALLERGY & ANAPHYLAXIS EMERGENCY CARE PLAN if your child has an EPI-PEN or requires medication while attending camp. *All Epi-pens and medications must be stored in our office and labeled with your child’s name in their original prescription labeled container.

*Students attending a FULL DAY session will bring lunch and only eat during our designated lunch hour. There are no limitations on packed lunches. Lunch staff will be alerted if a student has a food allergy.

Complex Medical Conditions, including Diabetes:

If your child has any known medical conditions or diagnosis (besides Asthma, Food Allergies, or a history of Seizures) please alert our office. If your child requires an ACTION PLAN for this known condition or diagnosis, complete the required form below. *Any medications must be stored in our office and labeled with your child’s name in their original prescription labeled container.

For Students with Type 1 or 2 Diabetes: Please provide us with your child’s DIABETES MANAGEMENT PLAN and complete the below Complex Medical Form.

Vaccination Records -OR- Religious Exemption Letters:

*Please provide us with an up-to-date vaccination record/chart or religious exemption letter prior to your child’s first day. Returning students must provide a new/updated record or letter each season. We DO NOT require COVID or FLU vaccines.

*Vaccination records or exemption letters may be faxed (732-223-0288) or emailed (siltonvax@gmail.com) to our office or mailed in along with your child’s registration prior to their first day of camp.
