Required Documentation for Current Employees:
All new staff must complete the above i9 their first year of employment. This document is only required during your initial hiring process.
Each year we require ALL staff to complete the above Emergency Medical Form and employee Code of Conduct.
For staff ages 17 & younger: Working papers must be completed your first year of employment. Please click the button above to complete your working papers online.
For NEW staff age 18 & older: Please click the button above to follow the steps to schedule your background check. Background checks only need to be completed once. For all staff who previously had a background check, and are over 18, please complete the below Affidavit.
Payroll Documentation:
Employees must complete these 3 documents their initial year hired. These documents will only need to be resubmitted if there are any CHANGES to your banking or w4s. Staff can view paystubs weekly by downloading and the ADP Mobile App. Make sure to sign up and register with the app. Once your informations is entered into ADP by Danielle, you will receive an email to complete your registration process.
